Which forms do new patients need?

New patients should complete the Wisconsin Consent, Medical History, Patient Info, and Informed Consent forms.

What if I cannot open the PDF forms?

You may need Adobe Reader®, which is free to download. You can also call (715) 644-3601 and we will help.

Should I update my forms at future visits?

Yes. Please let us know about any changes to your health, medications, or insurance so your records stay current.
